چکیده

purpose: to determine the dissection level during deep lamellar keratoplasty (dalk) using the big-bubble technique by light microscopy (lm) and transmission electron microscopy (tem). methods: descemet’s membrane (dm) and stromal tissue of 4 keratoconic eyes were removed because of inadvertent intraoperative dm perforation during dalk which was converted to penetrating keratoplasty. these specimens were divided in two halves, one half was fixed in 10% formalin for lm, and the other half was fixed in 2.5% glutaraldehyde for tem. results: lm examination of the corneal stromal and dm specimens, disclosed scattered round empty spaces consistent with air bubbles within the stroma, and pas-positive membrane with adherent endothelia on one side and scant amounts of posterior stroma including keratocytes on the other side. conclusion: separation of stromal corneal tissue from dm during dalk using the big-bubble technique for keratoconic eyes occurs within the deep stromal tissue near dm.

Small-bubble deep anterior lamellar keratoplasty technique.

Stromal disease (ectasia, opacities, scars, or melting) that occurs after penetrating keratoplasty (PK) can variously affect visual outcome.1,2 To date, even in the presence of healthy endothelium, this typeof complicationhasbeen treatedwith subsequentPK.3 Instead, the selective replacement of the diseased stroma by means of deep anterior lamellar keratoplasty (DALK) using pneumatic dissection ...

Easy Bubble: Pachymetry-assisted Big-Bubble Deep Anterior Lamellar Keratoplasty

We present a simple, effective and reproducible technique for deep anterior lamellar keratoplasty. Using a diamond knife, set at 90% of corneal thickness (90 degrees meridian), at the limbus a radial incision was made and a corneal pocket was created. A specially designed cannula was placed deep into the corneal stroma and air was injected for stromal dissection. The air bubble pushed down Desc...

Deep Anterior Lamellar Keratoplasty Using the Big-Bubble Technique in Keratoconus

PURPOSE During deep anterior lamellar keratoplasty (DALK), endothelium and Descemet's membrane are separated from the corneal stroma by intrastromal air injection ('big-bubble technique'). The aim of our study is to analyse histopathological changes in host corneal tissue caused by air insufflation in patients with keratoconus, their variability in 10 patients and their possible clinical implic...
